Important patient notice

We have now moved to an online triage system for all medical and admin requests. Please use our new online form. It is quick and safe and helps our clinicians prioritise care and direct you to the most suitable support. This change is part of a national NHS programme to improve access for patients. We know that change can be difficult and we are all getting used to this new system. Sadly, since moving to online triage, our reception team has faced unacceptable behaviour from a small number of patients. Our reception and administrative teams are here to help you, so please speak to them with respect and kindness.

Woosehill Medical Centre has a zero tolerance policy for abusive or rude behaviour towards our staff.

Confirming your home address with the NHS England

You may receive a message from the NHS asking you to confirm your home address. This is a routine check carried out by NHS England to make sure your GP registration details are up to date.

You may be contacted by:

  • Email from nhs.confirm.your.address@notifications.service.gov.uk
  • Letter
  • Text message from NHSdetails

Why you may be contacted

The NHS checks patient addresses from time to time. This helps keep records accurate and makes sure patients remain registered with the correct GP practice.
If you do not confirm your address when asked, you may risk being removed from your GP list after a set period of time.

The email address above is used by the NHS for this purpose. It is a real address.
If you are ever unsure, do not click on any link. Please contact the practice and we can check this for you.

What you need to do

  • If your address has not changed, follow the steps in the message to confirm it
  • If you have moved, please let us know so we can update your record
  • You can also update your address through your NHS App, Change your home address – NHS or by contacting us

If you do nothing

If the NHS does not receive a response, you may be removed from your GP practice list. This means you would need to register again to receive routine GP care.
