Over 16’s consent form for proxy access

If you would like to give someone else (for example, a parent, carer, or relative) permission to access your online medical record or manage your appointments and prescriptions on your behalf, you can request Proxy Access online

You can use this service if you:

  • are registered with Woosehill Medical Centre, and
  • are aged 16 or over

Before you start

We’ll ask you to provide:

  • Your full name, date of birth, sex, and postcode
  • Your email address and phone number
  • The name, date of birth, and contact details of the person you wish to give proxy access to
  • The level of access you’d like them to have (for example, appointments only, prescriptions, or full record access)
  • A photo ID (such as a passport or driving licence) and proof of address (such as a utility bill) for both people

🔒 Important:
By giving proxy access, you’re allowing another person to view or manage your health information.
You can change or remove their access at any time by contacting the surgery.

Once you submit, we will review and may contact you if further information is needed.
