Private prescription for drugs: £18.00
Private sick note: £30.00
Private Insurance forms: £30.00
Private consultation (10 minutes): £40.00
Private consultation – Nurse: £28.00
Fitness to travel certificate: £30.00
Holiday Cancellation certificate: £30.00
OFSTED Reports for Childminding: £50.00
Copy of GP Computerised Summary Records: £10.00
Copy Notes – paper records (Maximum): £10.00
Medical Examinations: £130.00
e.g. pre-employment, taxi driver etc
ECG: £50.00
Hepatitis B vaccinations (course of 3): £39.00 per vaccination
