Why you may be contacted
The NHS checks patient addresses from time to time. This helps keep records accurate and makes sure patients remain registered with the correct GP practice.
If you do not confirm your address when asked, you may risk being removed from your GP list after a set period of time.

What you need to do
- If your address has not changed, follow the steps in the message to confirm it
- If you have moved, please let us know so we can update your record
- You can also update your address through your NHS App, Change your home address – NHS or by contacting us
If you do nothing
If the NHS does not receive a response, you may be removed from your GP practice list. This means you would need to register again to receive routine GP care.
