Woosehill Medical Centre only accept requests from our own registered patients.

Our nursing team is able to provide patients with a comprehensive travel advice and vaccination service. If you are travelling abroad please complete and submit the travel form in plenty of time (we recommend 6 weeks) before your journey.

One form is required for each patient travelling.

Once completed a nurse will then assess your needs and record recommendations on the computer. While it is hoped this assessment will be made within 2 weeks this cannot be guaranteed.

When your form has been assessed you will receive a text advising you of vaccines required and the length of appointment needed. Please phone reception to book the appointment.

Alternatively, you can download the form to fill in by hand.

All vaccinations must be paid for on the day and before seeing the Nurse.
