Register for online services

You can now access a variety of services online without having to phone or visit the surgery. Sign up now to GP online services and you will be able to use a website or app to:

Online services allow you to:

  • Order repeat prescriptions
  • View test results where appropriate
  • View parts of your GP health record
  • Check or cancel appointments

Using the NHS App

The easiest way to access your GP services is by using the NHS App.

The NHS App uses NHS Login to securely verify your identity and allows you to access your GP record, prescriptions and other NHS services.

If you are able to use the NHS App, you usually do not need to register through the practice.

Download the NHS App or access your NHS account online to get started.

Registering through the practice

If you are unable to use NHS Login, you can request online access directly through the practice using the form below.

All requests must be made in writing and identity verification may be required before access is granted.

You may be asked to provide identification such as:

  • Passport
  • Driving licence
  • Utility bill
  • Bank statement

Once your identity has been verified, access will be enabled on your GP record.

Access to your medical record

In most cases, patients will automatically be able to see new information added to their GP record through online services.

Some information may be restricted where this is necessary to protect patient safety or third party confidentiality.

The practice may delay, limit, or remove access if there are safeguarding concerns or if disclosure could cause harm.

Proxy access

Proxy access allows another person to manage a patient’s GP services on their behalf.

This may include:

  • Parents managing access for children
  • Carers supporting a patient
  • Someone with legal authority such as lasting power of attorney

Please consider

We would encourage you to consider the below, before enabling online 24/7 access to ensure this is a service helpful to you.

  • Forgotten history – There may be something you have forgotten about in your record that you might find upsetting.
  • Abnormal results or bad news – If you have access to test results or letters, you may see something that you find upsetting to you. This may occur before you have spoken to your doctor or while the surgery is closed and you cannot contact us.
  • Coercion – If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.
  • Complex information –  Your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care. Some of the information within your medical record may be highly technical and not easily understood. Further clarification can be sought from the Practice, ideally requests are submitted in writing or via e-mail with clear reference to the text or terminology in question.
  • Information about someone else – Before access to your record is enabled, we must remove any reference to a third party. If you spot something in the record that is not about you or notice any other errors, please log out of the system immediately and contact the practice as soon as possible.

Please note

If you would like to register for online services (to order repeat prescriptions, book appointments, etc.), please note the following:

  • You will be asked to provide proof of ID. We will send you a secure link where you can upload this.
  • Once your ID has been verified, we will issue your online access documentation. This may take seven working days or longer.
  • All applications are carefully checked to ensure they are genuine.
  • For patient safety, access may be withdrawn at any time if there are concerns about confidentiality.

Please note that each family member requires an individual login and password, therefore a separate registration. This service is only available for patients aged 16 or over.

Before you start

We’ll ask you to provide:

  • Your first and last name, date of birth, sex, postcode, email address, and phone number
  • If you are completing the form on behalf of someone else, their details: name, date of birth, contact details, and relationship to you
  • The level of access you (or they) wish to grant (e.g. appointments only, prescriptions, or full access to online medical records)
  • Photographic identification (e.g. passport or driving licence) and proof of address (e.g. utility bill) for all parties involved
