Important patient notice

We have now moved to an online triage system for all medical and admin requests. Please use our new online form. It is quick and safe and helps our clinicians prioritise care and direct you to the most suitable support. This change is part of a national NHS programme to improve access for patients. We know that change can be difficult and we are all getting used to this new system. Sadly, since moving to online triage, our reception team has faced unacceptable behaviour from a small number of patients. Our reception and administrative teams are here to help you, so please speak to them with respect and kindness.

Woosehill Medical Centre has a zero tolerance policy for abusive or rude behaviour towards our staff.

Named GP

All patients registered with Woosehill Medical Centre have a named, accountable GP. This is a requirement of the NHS GP Contract and helps to make sure your care is well coordinated.

What this means for you

  • Every patient (including children) has a named GP recorded in their notes.
  • Your named GP is responsible for overseeing your care, but your care is provided by the whole practice team.
  • You can continue to see any of our GPs or nurses – you are not restricted to your named GP.
  • Your named GP is not available 24 hours a day. Out-of-hours services are provided by NHS 111 and other local providers.

Can I choose my named GP?

Yes. If you have a preference for a particular GP, please let our reception team know. We will make reasonable efforts to accommodate your request.

How can I find out who my named GP is?

You can:

  • Ask at reception
  • Check during your next appointment
  • Contact us via our online request forms

Patients aged 75 and over

Patients aged 75+ also have a named GP who will:

  • Work with community and social care services to coordinate care
  • Ensure access to an annual health check
  • Help plan ongoing support for long-term conditions
